Anyone who lives in the South Central Library System service area can apply for a free library card valid at all 65 libraries in the system. If you already have a card from another member library, there is no need for a new card.
To apply:
- Request an application form at the library or complete at home: Library Card Application Form
- Once the on-line form is submitted, you will receive an email indicating the form has been received. Please stop by the library after 2-3 days to pick up your new library card. (Please bring photo ID with you.)
- Acceptable proof of address: utility bill, car registration, bank statement, check, renter's agreement, closing documents, etc.
- Children under 16 must be accompanied by a parent or legal guardian and come to the library in person to sign the registration form and present a photo ID and proof of address.
Lost/expired cards or forgotten PIN: Please call the library at 608-838-9030 or visit the main desk for assistance.
Address changes: Please bring in photo ID and proof of address if your ID has not been updated accordingly (see examples above).
